Notes On Completing the Form:

  • Double check your entries to ensure accuracy before you submit this form. You will not be able to edit your entry after it has been submitted.
  • Any changes to your entry registration after submission will have to be coordinated with the Tournament Director. (Dan Porter – 734 308-9453)
  • When making your selection in the Cost Calculator, do not include anglers under 17 years of age in your count.
  • Ensure that your selection in the cost calculator agrees with the total number of team members you have listed on the form. Any mismatch may result in your registration being declined!
  • Your total cost will be calculated based on $30.00 per team member.
  • Once you successfully submit your registration, the “Captain” will receive an emailed copy of the entry record. A separate emailed verification of the credit card transaction will be sent to the email address entered in the CC transaction field. The latter comes from the credit card processor (Stripe). The DWF will not see any details of your secure CC transaction.

  • To be accepted, this registration must be submitted no less than 36 hours prior to the start of the selected tournament!
DWF 2026 Tournament Registration Form
This form is used to register a Captain/Team to participate in a Downriver Walleye Federation Tournament. One form submission is required for each tournament event. The cost will be $30.00 per registered angler.
PLEASE double check that you have selected the correct event from the pull down menu.
The Team Name you enter will be used to identify all team members by event.
Captain's Name
IMPORTANT NOTE: The Captain Must be a DWF Member in good standing. Otherwise, the registration will be declined.)
The Captain will receive a copy of the completed registration at this email address.
Please include the vessel registration number and a basic vessel description. (Example: 2001 Lund Pro-V. Red with a 200HP Merc.) This field is not required, but is recommended.

Liability Release

In signing this agreement, I hereby release the Downriver Walleye Federation, its officers, and members, its sponsors and their agents (hereinafter referred to as D.W.F., et all) from any and all damages, claims, demands, costs or expenses relating to injuries, death, damage, theft, fire or any loss of any persons or any property which I may sustain and or which I may cause by reason of participating in or in connection with this tournament. I further release DWF, et all for all damages relating to adverse weather conditions while participating in this tournament. It is my responsibility to seek safety when adverse and/or unforeseen weather conditions occur prior to, during or after the tournament.

By signing below, I also agree that each team member signed up on this registration form for this event has read, understands and agrees to this liability release.

I understand and agree that this release is intended to be as broad and inclusive as permitted by the laws of the applicable jurisdiction. If any portion is held invalid, the remainder shall continue in full legal force and effect.
